Appointments:
Our office sees patients by appointment only. We have our schedules available for 3-4 months in advance in order accommodate check-ups and follow up care. We leave room in our daily schedule for same day sick appointments. We know that your time is valuable and we try to stay on schedule, but other patients’ unforeseen emergencies may delay your visit. If you need to cancel an appointment, we would appreciate 24 hours notice so that we can offer the time to another patient.
Immunizations:
We follow the AAP recommended immunization schedules. View our Immunization Policy and other important information here.
Financial Policy:
We expect payment at time of service for co-pays, coinsurance, and deductibles for plan members. View our full Financial Policy here.
Phone calls:
We have a nurse advice line available during office hours to assist you. The nurses that handle these calls are the same nurses that care for your child at our office. Calls are normally returned within 1-2 hours of your message; messages left after 4:45pm will be returned the following business day.
After Hours/Emergencies:
In the event of a life-threatening emergency, you should call 911 or proceed to the nearest emergency facility. You can reach us after hours by calling our regular number – 770-751-6111 and following the prompts. View our full After Hours/Emergency procedure here.
Medical Records:
The privacy of your child’s medical record is of the utmost importance to us. We will only release records to non-medical requestors such as insurance companies or law firms with your written permission. View our full Medical Records Policy here.
Forms & Letters:
There will be no charge for forms completed when a patient is seen for an office visit or regular checkup, for forms that a family has prepared that only require the physician’s signature, or for prescriptions that are for previously prescribed medications (refill prescriptions). - Patient Information Sheet - Attention New Patients - This form is now done in the office on one of our iPads. Please come in 20 minutes before your scheduled appointment so we can get your child in our system and keep them on time for their first appointment with us.
